What Personal Information Does Artemis Collect?

Artemis does not ask you to provide personal health care information through our website (www.arianadx.com).

Identifying Information: Artemis will collect information that identifies you if you voluntarily choose to provide such information. If you choose to register with our site, we may ask you to provide some personal information such as your first and last name and contact information (including mailing address, telephone number, and email address). We may also collect information that will allow you to create a username and password.

Payment Information: If you choose to monitor and pay Artemis invoices electronically by clicking on the Artemis Bill Payment Link, you will be asked to provide personal financial information (including credit card number and billing address). Clicking on the Artemis Bill Payment link does not display or transmit any of your personal health care information.

How Does Artemis Use the Personal Information We Collect Online?

We may use your personal information within Artemis to (1) provide you access to the services and products you request; (2) process or collect payments made in connection with the services we provide to you; (3) assist you with questions about our services, billing, payment methods, or use of our website; and (4) evaluate our services and products and conduct customer surveys.

We may also use your personal information within Artemis, with your consent, to send you information concerning Artemis products and services that might be of interest to you. How Does Artemis Protect the Security of Your Information?

We limit Artemis employees’ and contractors’ access to personal information. Only those employees and contractors who have a business reason for accessing the information will have such access. We educate our employees on the importance of confidentiality and customer privacy through internal training programs and policies on data privacy and security. Further, we take appropriate disciplinary measures to enforce our privacy rules and regulations.

Artemis maintains all personal health information within the Artemis information system firewalls that operate on separate Artemis mainframes/servers. The general public does not have access to these mainframes/servers.

Please use caution when sending personal information via email; email messages do not have the security features that are built into our website.

Cookies

Artemis tracks visitors to our website by collecting data files called “cookies”. Cookies collected from the Artemis website are used to: 1) let us know that you are a prior customer and retrieve information previously provided by you; 2) identify which computer you are using; 3) monitor website traffic; and 4) help us gather feedback in order to improve our website and better serve our customers.
